Prepare for a WIC Appointment
When you are part of the WIC program, there are many different types of appointments to attend. Our staff want to guarantee that your appointments are smooth and successful so that you get everything you need throughout your journey. Please read through the types of appointments below to see what you'll need to bring to your next appointment.

Types of Appointments
Initial Certification πΆππ₯
This in-person appointment is your first WIC visit.
Documentation Needed: Please take photos of the following items and email them to wic@clayhealth.com at least 48 hours before your appointment. If you are unable to send them in before, please bring all items to your appointment and try to arrive 15 minutes early.
- Form of ID (identification)
- Child
- Immunization Record (preferred) OR
- Birth certificate or hospital document
- Adult
- Driver's license or passport OR
- Photo ID card (state, work, school)
- Child
- Proof of income within 30 days
- 1 month of paystubs 30-60 days before appointment OR
- Previous year's W2 or Federal Tax Return OR
- Any other income source documentation
- Proof of residence within 30 days (statement or billing date must be within 30 days before appointment)
- Utility or personal bill/paystub OR
- Bank statement/rent or mortgage receipt OR
- Social services awards letter OR
- Voter registration card OR
- Property tax receipt
Needs to Attend: The child participant with their guardian
Re-Certification πΆππ₯
Your certification is due for renewal after 1 year. (Unless prenatal – cert happens at prenatal to postpartum transition.) The baby MUST BE at least 12 months old; must be on their birthday or after)

- Proof of income, dated within 30 days
- Proof of residency, dated within 30 days
- Birth certificate or hospital record or immunization record

Mid-Certification πΆπ₯
This is an in-person appointment that happens mid-way through certification, where measurements are taken.

Prenatal Certification ππ₯
This is an in-person, initial certification with your current pregnancy.
Documentation to Bring or Send in Before: Please take photos of the following items and email them to wic@clayhealth.com at least 48 hours before your appointment. If you are unable to send them in before, please bring all items to your appointment and try to arrive 15 minutes early.
- Form of identification (ID)
- Proof of income, dated within 30 days
- Proof of residency, dated within 30 days
Needs to Attend: The pregnant person getting certified
Nutrition/Tele-Nutrition π
This is a follow-up with your nutritionist, most often over the phone but can be in person.
Documentation Needed: none
Needs to Attend/Call: The adult participant or guardian of the child participant
Group Nutrition Class π₯
This is an in-person class and counts as a nutrition contact.
Documentation Needed: none
Needs to Attend: The adult participant or guardian of the child participant
Breastfeeding Class π₯
This is an in-person class and counts as a nutrition contact.
Documentation Needed: none
Needs to Attend: The breastfeeding participant. The breastfeeding child(ren) and/or one support adult is welcome to attend as well.
Breastfeeding Appointment π
This is an appointment with the Breastfeeding Peer Counselor, often over the phone.
Documentation Needed: none
Needs to Attend/Call: The breastfeeding participant
eWIC/Card Loading (Check Pickup) π₯
This is a short appointment where we will load benefits to your eWIC card at the office.
Documentation Needed: none
Needs to Attend: The adult participant or guardian of the child participant
6-Month Assessment πΆπ₯
This is an in-person appointment midway through your infant’s certification period, where measures are taken. The child MUST be AT LEAST six months old.
Documentation Needed: none
Needs to Attend: The infant participant with an adult guardian. Mom's mid-certification appointment can take place during this appointment if mom is mostly/fully breastfeeding.
9-Month Hemoglobin πΆπ₯
This is an in-person appt with your nutritionist, where your infant’s hemoglobin will be measured with a fingerstick. The child MUST be AT LEAST nine months old; must take place on or after their nine-month birthday.

Needs to Attend: The infant participant with an adult guardian
Postpartum Hemoglobin πΆπ₯
This is a nutrition appointment in-person where your nutritionist will check your hemoglobin. You must be at least 4 weeks postpartum.
