Community Health Improvement Plan (CHIP)

After the Community Health Assessment is complete, an updated Community Health Improvement Plan (CHIP) is created.

In the Northland Health Alliance, three top priorities to focus on for the next three years are chosen, based off the findings of the CHA. NHA's three task forces are then assigned to each of the priorities and must help come up with plans to make improvements in the community. These plans are then compiled into the Community Health Improvement Plan and usually published about six months after the CHA.

The CHIP states the groups’ goals, strategies, what actions should be taken, a timeline and more. It also includes key performance indicators, which are ways the task forces will know they’re on their way to meeting their goals.
