Birth Certificates

Clay County Public Health Center can print certified copies of original birth certificates for individuals who were born in the state of Missouri after 1920.

Death Certificates

Clay County Public Health Center can print certified copies of original death certificates for individuals who were deceased in the state of Missouri after 1980.

Fees

  • Birth Certificates: $15 per copy
  • Death Certificates: $14 for first copy, $11 for each additional copy per decedent

There are multiple ways to get certificates from Clay County Public Health Center. See below for more details.

Birth and death certificates are available at the health center from 8 a.m. - 4:30 p.m. Monday to Friday, excluding holidays. Same-day service is available to walk-in customers. You must have a photo identification card of some kind, e.g. driver's license, work I.D., school I.D., etc. to obtain the copies.

You can fill out the application in advance: 
Application for Certified Copy of Birth/Death Certificate for the State of Missouri. Please print the document and bring it when you come; otherwise, the form will be provided when you arrive.

*For Older or Out-of-State Certificates
